The care manager will also be involved in doctor’s visits, medical advocacy, appropriate resources, social engagement, travel companionship, and interaction with medical providers. The program includes access to a world-class referral network and/or interactions with aligned professionals, such as financial planners and attorneys. The goal is to ensure that communication flows among all professionals associated with the client, and the care manager can pivot to provide as much involvement as needed as the situation changes.

The Solo Agers Program also provides advice on alternative living arrangements, remaining in the home, services, and resources based upon the needs of the moment. The care manager remains one phone call away and is always available to help clients and their families.
